Client for this thesis is Kotkan Työterveys Oy, which is an occupational health service in Kotka, Finland. Their wish was to get a new preliminary information sheet for pre-employment examination. The goal of the pre-employment examination is to evaluate whether the employee is fit for the job, when there are risks involved in the job. In cases when the work doesn’t have any risks the purpose of the pre-employment examination is to evaluate if the work suits for the new employee. The preliminary information sheet contains questions about the employee’s work, the employee’s ability to work, the employee’s health and the employee’s life style. The employee fills in the preliminary information sheet and brings the fulfilled version to the public health nurse at the pre-employment examination.

The aim of this thesis was to produce a preliminary information sheet in Finnish and in English. The short term goal was to produce a preliminary information sheet, which fulfills the needs of the public health nurses. It could also be used as a good aid and support the progression of the pre-employment examination. The employees of customers of Kotkan Työterveys Oy will find the examination sheet in Kotkan Työterveys Oy’s internet page and they can use as much time as they need in filling it. A thoroughly fulfilled preliminary information sheet contributes the pre-employment examination progression and customer-friendliness. The long-term goal was to support employee’s health and ability to work. My goal for this thesis was to increase my knowledge about the objectives, process and services in occupational health care. Another goal for me was to develop my skills in project work and to improve my communicating and organizing skills.

Before I started to develop the preliminary information sheet I had to familiarize myself with the scientific knowledge about history, legislation, service process and contents of the occupational health care. I paid extra attention to the public health nurses work in the health examination. The theory portion of my thesis was based on these subjects. The preliminary information sheet I produced is based on the knowledge about occupational health care, Kotkan Työterveys Oy’s hopes for the preliminary information sheet and the goals I made for the preliminary information sheet.

My suggestion for further research is to produce a guide, which helps the employees to understand the meaning of exposures and helps the employees to avoid the exposures in work. The preliminary information sheet would also be useful in Russian. The preliminary information sheet could also be transformed to an internet version, so that the employees could fill the sheet and send it to the occupational health care via internet.
